Hi. I would do it like this:

Mr. and Mrs. John Doe

and

Mr. and Mrs. John Doe and Family

It is Mr. and Mr. John Doe. On the inner envelope is where you put and family.

It doesn't matter officially that I know of. I am addresing mine differently depending on who they are goin to. For example, my brother and his wife I wrote: Mr. and Mrs. John Doe since she is totally happy and excited to be the "wifey" type. For my fiances brother and his wife I wrote Mr. and Mrs. John and Jane Doe since she is more the type who "wears the pants in the relationship." I would say a safe bet is to address them to Mr. and Mrs. Doe.

It is whatever you want it be, whichever looks best to you. If you are having a really formal wedding I would go with the 3rd option, otherwise #2 would is nice too. I think #1 is a little lengthy though. Good luck and congrats!

Mr and Mrs John Doe

If you are inviting a single and their guest:

Mr. Smith and Guest

If you are inviting a couple that is not married:

Mr. John Smith and Ms. Jane Doe

Mr. & Mrs. John Doe, unless they use different last names: then it would be Mr. John Doe and Mrs. Jane Smith.
